Long Term Care Choices Expand –
Nursing Home Transition Offered
The B/S/S/T Area Agency on Aging is pleased to announce the availability of a new program that provides information and assistance to those nursing home residents age 18 and older who may be interested in transitioning into the community. Nursing Home Transition serves nursing home residents of all income levels and is a collaborative effort directed by the nursing home resident.
Some Nursing Home Residents may be eligible to receive Waiver Services in the community through a variety of different Waiver Programs funded by Pennsylvania Medical Assistance. Liberty Resources of Allentown, PA serves individuals under the age of 60 and is a collaborative partner with the B/S/S/T Area Agency on Aging for the Nursing Home Transition Program in the counties of Bradford, Sullivan, Susquehanna, and Tioga.
A Nursing Home Transition Coordinator visits residents who have expressed an interest in returning to the community to provide information on available housing options, in-home services, and other resources of help. The resident directs the Nursing Home Transition Coordinator and members of a transition team to plan for his/ her return to the community.
The Transition Team includes Nursing Home Staff members, family members, and other individuals of the consumer’s choice. The team discusses the residents’ care needs and his/her options for receiving care in the community and may advise the resident about the feasibility of returning to the community with in-home services.
If the resident decides that receiving care in the community will adequately meet his/her care needs, the Nursing Home Transition Coordinator coordinates all transition arrangements including housing, in-home services, moving and other arrangements.
