Pre-Admission Assessment

Pre-Admission Assessment is a service that helps determine the appropriate level of care for individuals age 18 or older who need a supplement to help pay for their expenses in a long term care facility. Options Assessors visit individuals in their homes, in hospitals, in Personal Care Homes, Dom Care Homes, or Nursing Homes. The Assessors complete an Assessment which reflects the individual's specific care needs.
A level of care such as Nursing Home or Personal Care Home is identified from information gathered in the Assessment process. There is input from the individual's physician and the information is reviewed with the agency's Nurse Consultant to determine the appropriate level of care that will best suit the individual's overall needs.

Our Care Management staff will assist individuals who desire to relocate to a long term care facility such as a Personal Care Home, Nursing Home or Dom Care Home. The agency offers listings of all licensed long term care facilities in the counties of Bradford, Sullivan, Susquehanna, or Tioga and also information on how to choose an appropriate home.
