Our Professional Care Managers visit adults who are age 60 or older at home to explain available resources of help. They may complete a comprehensive assessment of needs and coordinate appropriate In-Home Services. They often can provide guidance about available options and resources. Care Management is a service available free of charge to individuals of all income levels.

Community Services
Community Services like Home-Delivered Meals, Personal Care, Emergency Response Systems, and Home Support help older individuals to remain in their homes. Cost-sharing applies in some situations, but is usually very affordable. Other services available to eligible older citizens include Legal Services, Transportation Services, and Employment Services.
