Care Management

Our Professional Care Managers visit adults who are age 60 or older at home to explain available resources of help. They may complete a comprehensive assessment of needs and coordinate appropriate In-Home Services. They often can provide guidance about available options and resources and can assist with entitlement and benefit applications. Care Management is a service available free of charge to individuals of all income levels.

Anyone may request services for themselves or another person. Referrals are received and welcomed from family members, friends, neighbors, health care professionals or anyone who recognizes the need for assistance and services. A telephone call to the Area Agency on Aging, 1-800-982-4346, will begin the referral process.

Care Managers also make regularly scheduled monthly visits to Senior Community Centers in the service area to provide information on services offered by the Area Agency on Aging and assist with completing applications and forms for additional services such as LIHEAP, Food Stamps, Property Tax Rent rebates and PACE.

Community Services

Community Services like Home-Delivered Meals, Personal Care, Emergency Response Systems, and Home Support help older individuals to remain in their homes. Cost-sharing applies in some situations, but is usually very affordable. Other services available to eligible older citizens include Legal Services, Transportation Services, and Employment Services.
